Creative Health Care Management — Website Privacy Policy

Creative Health Care Management (CHCM) collects information about its clients for the purposes of ongoing consulting work and team and leader education. This Privacy Policy explains how we collect, use, and share information when you visit chcm.com, including our online store at chcm.com/shop (the “Site”). We do not sell, rent, or transfer client data to any outside organization for money.
1. Information We Collect
Information you provide to us. Information you choose to give us when you complete a form, request information, place an order in our store, subscribe to communications, or contact us — such as your name, title, organization, billing and shipping address, phone number, email, communication preferences, order and purchase history, and the content of your messages.
Order and payment information. When you purchase from our store at chcm.com/shop (powered by WooCommerce), we collect the details needed to process and fulfill your order, including your billing and shipping details and order contents. Payment card details are processed by our payment provider; we do not store full card numbers on our servers.
Information collected automatically. When you visit the Site, some information is collected automatically through cookies and similar technologies, such as your IP address, device and browser type, the pages you view, and how you arrived at and moved through the Site.
Business/organization identification. We use a visitor-identification service that uses your IP address to identify the organization associated with your visit, so we can understand which businesses are interested in our services.
2. Cookies and Tracking Technologies
We use cookies and similar technologies to operate the Site, understand how it’s used, run the store, and support our communications:
Strictly necessary — required for the Site to function, including WordPress authentication and the WooCommerce shopping cart and checkout session. Always active.
Analytics and performance — Google Analytics 4 (GA4), to understand how the Site is used. (We also use Ahrefs for SEO; it does not set cookies or collect personal data on the Site.)
Call tracking — CallRail, to understand which pages and sources lead to phone calls; it may capture your phone number and call details.
Email communications — Constant Contact, which supports our newsletters and email communications and may set cookies related to sign-up and email engagement.
Business identification — ZoomInfo, described above.
The WooCommerce cart and checkout cookies are strictly necessary and always active. Other non-essential cookies (analytics, call tracking, email, and business identification) load only after you consent through our cookie banner. You can change your choices at any time using the “Cookie Settings” link in the footer.
3. How We Use Information
We use the information described above to respond to your inquiries and provide the information, products, or services you request; to process and fulfill store orders; to operate, maintain, and improve the Site; to send communications about new products, service enhancements, program development, and select co-sponsored events we believe are valuable to you; to understand which organizations are interested in our services; and to comply with legal obligations and protect our rights.
4. How We Share Information
We share information only as described here:
• Service providers who perform functions on our behalf — such as website hosting, analytics, call tracking, email delivery, visitor identification, order fulfillment, and payment processing for the store — under agreements that limit their use of the information.
• Legal and safety — when required by law or to protect rights, safety, and security.
We do not sell your personal information for money.
5. Your Privacy Rights
California residents have rights under the CCPA/CPRA, including the right to know, access, correct, and delete personal information, to limit the use of sensitive personal information, and to opt out of the sale or sharing of personal information. Use the “Cookie Settings” control or send a Global Privacy Control signal to opt out.
Minnesota residents have rights under the Minnesota Consumer Data Privacy Act, and residents of other states with comprehensive privacy laws have similar rights, including to access, correct, delete, and port their personal data and to opt out of targeted advertising and the sale of personal data.

6. Global Privacy Control
We honor the Global Privacy Control (GPC) browser signal as a valid opt-out of the sale or sharing of personal information where required. The Site does not separately respond to “Do Not Track” signals.
7. Data Security
Keeping data secure is a top priority for CHCM. Personal and financial records are handled in accordance with PCI-DSS practices. We use 256-bit encrypted SSL certificates across our Internet properties, including our WooCommerce store at chcm.com/shop, and Extended Validation certificates on our highest-volume properties. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

If you wish to be removed from our contact list, call our office at 800.728.7766 or 952.854.9015, email chcm@chcm.com, or click the “Unsubscribe” link at the bottom of any of our email communications. To correct or request removal of other information we have about you, contact chcm@chcm.com.
